That was my initial fear too (that the line referred to someone else who loved her besides Rumple), but other interpretations of that line make me think I was reading too deeply into it the first time. Taken in context with the line before it, it seems safer to think that they're actually referring to Belle somehow seeing Gold while in the hospital and not realizing who he is, either because she only saw his non-cursed form briefly or because she lost her memories of the fairy tale world.
